Code: Select all

<?
if (isset($_POST['subject'])) {
  $from = $_POST['from'];
  $subject = $_POST['subject'];
  $message = $_POST['message'];

  if (!$from||!$subject||$message) {
    echo "Not all Fields were Filled In!";
  }else {
    mail('myemali@example.com', $subject, "From: ".$from." \n".$message);
  }
}else {
  display form, action being $PHP_SELF
}
?>

Untested, just add form and should work
